Holy Redeemer and Prepared Health Partner to Connect Health System, Align Post-Acute Network

After successful partnership with its Home Care unit, the Pennsylvania-based health system adds Hospital and Life Care Centers to Prepared Health's digital enTouch network

PHILADELPHIA, Feb. 5, 2019 /PRNewswire/ -- Holy Redeemer Health System, a fully integrated health system serving patients across Pennsylvania and New Jersey, has expanded its partnership with Prepared Health's enTouch network, a digital platform to help hospitals build, digitize and manage post-acute partners.

Holy Redeemer will leverage enTouch technology to connect its Hospital, Life Care Centers and Home Care agencies, and organize and empower its preferred post-acute, home and community-based care providers. With enTouch, post-acute partners can collaborate in real-time to eliminate communication gaps that often occur when multiple providers are involved with patient care post-discharge.

"As care continues to move outside of the hospital, innovative leaders like Holy Redeemer are stepping up to ensure patients receive quality care as they move to a post-acute or home setting," said Tim Coulter, COO of Prepared Health. "Holy Redeemer is an outstanding partner in our mission to reduce avoidable hospital readmissions and drive better patient-centric data-driven experiences on transitions from hospital to home."

Holy Redeemer also will have access to DINA, Prepared Health's Digital Nursing Assistant, which analyzes patient data and uses artificial intelligence to identify and recommend evidence-based interventions and improve post-acute transitions.

"We are excited to partner with Prepared Health in our mission to provide the highest quality of care to our home care and hospice patients," said Patricia O'Brien, Sr. VP/Chief Administrative Officer at Holy Redeemer Home Care & Hospice. "The technology supports our commitment to safe transitions in care and open, consistent communication with our referral sources and physician partners. This technology allows our team to refer and accept patients for care, update our care partners on patient status, and work collaboratively to prevent unnecessary ER visits and hospital admissions."
