Connected Care Launches Excelera Accountable Care Organization (ACO), Expands Commitment to Consistent Quality Care

Excelera ACO brings providers of services, provider groups and suppliers together into an ACO to promote accountability and coordination for a population of Fee-For-Service Medicare beneficiaries.

NEWPORT BEACH, Calif.--(BUSINESS WIRE)-- Connected Care (www.cchealth.com/), the world’s first global Payvidance™ platform, announced one of its subsidiaries -- Excelera ACO -- was selected by The Centers for Medicare & Medicaid Services (CMS) to participate in a Medicare Shared Savings Program as an accountable care organization (ACO), basic track A model. The initiative launched July 1, 2019.

Excelera ACO brings providers of services, provider groups and suppliers together into an ACO to promote accountability and coordination for a population of Fee-For-Service Medicare beneficiaries. Excelera ACO moves away from volume-based to value-based care delivery by promoting coordination of care, high-quality care, efficient service delivery and patient-centricity as the only long-term, sustainable solution.
