RelayHealth Launches New Patient Care Management Tools

EMERYVILLE, Calif., Oct. 10 /PRNewswire/ -- RelayHealth Corporation, the nation’s premier provider of health plan-reimbursed online doctor-patient communication services, today announced the release of new capabilities that simplify and streamline both targeted and scheduled outreach to patients.

RelayHealth’s new Patient List Builder allows clinical users to dynamically profile patient by characteristic -- including gender, age, clinical diagnosis, or medication as recorded in their RelayHealth Personal Health Record -- and then view, download, print, or securely message target patients. This feature enables practices to communicate important, targeted health information electronically, at virtually no cost. For example, preventive exam or immunization reminders can be distributed to targeted patients securely online, in accordance with clinical care guidelines or pay-for-performance program criteria. Educational materials supporting care plan adherence can be sent to patients managing a chronic illness such as diabetes. Information regarding drug recalls or label changes can be sent almost instantly to all patients whose PHR indicates active use of the affected medication.

RelayHealth has offered patients a free, portable online Personal Health Record (PHR) since the service launched in 1999. The RelayHealth PHR is updated each time patients communicate online with their physicians, and is automatically populated with medication records each time their physician writes and transmits a prescription using RelayHealth’s eScript(R) electronic prescribing service.
