Texas Residents Sought For Medicare Demonstration Project - XLHealth To Support Physicians In Treatment Of Chronically Ill - - Comprehensive Prescription Drug Coverage Offered -

BALTIMORE, May 24 /PRNewswire/ -- XLHealth Executive Vice President Paul Serini announced today that the company is seeking qualified Texas residents for a Medicare Disease Management Demonstration project that supports physicians in their treatment of the chronically ill. The program supplements a doctor's care and offers comprehensive prescription drug coverage effective immediately.

The Medicare demonstration project will involve Medicare patients in the cities and surrounding communities of San Antonio, Corpus Christi, El Paso, Austin and Dallas/Forth Worth. In order to qualify, residents must be Medicare patients living in these areas with a diagnosis of congestive heart failure or complex diabetes (including diabetes with lower extremity complications and advanced congestive heart failure or cardiovascular disease). There are approximately 130,000 Texas residents in these areas that potentially qualify.

XLHealth, a disease management firm, was selected by the United States Department of Health and Human Services Centers for Medicare and Medicaid Services (CMS) to conduct a demonstration project aimed at improving care for chronically ill Texas Medicare patients through disease management.

The firm anticipates that disease management will dramatically improve the ease of obtaining care, and health care outcomes, for the seriously ill Texans who are part of its Medicare Disease Management Demonstration project. Based on XLHealth's work with physicians and their chronically ill patients in such Texas communities as San Antonio and El Paso, the firm expects that in aggregate, the approximately 10,000 patients enrolled in the XLHealth program will see a 50 percent reduction in amputations and other serious diabetes foot complications, and a 50 percent decrease in repeat heart failure events.
