Newly refined methodology better assists agencies in preventing hospitalizations
SANTA BARBARA, Calif., Feb. 20, 2019 /PRNewswire/ -- Strategic Healthcare Programs (SHP), the leading data analytics and benchmarking to post-acute care providers, has enhanced its predictive analytics for identifying home health patients that are at-risk of being hospitalized. SHP made the decision to make significant enhancements and refine the methodology with the implementation of OASIS-D in January 2019.
SHP’s new model now uses a significantly increased number of risk factors to improve the prediction accuracy and reduce the percentage of false positives. In comparison to the previous model, the new model boasts a higher “true positive” and a lower “false positive” rate across the continuum of risk.
SHP has not only improved the model’s accuracy but is now able to identify risk levels at a more granular level, rather than just moderate and high risk levels. Based on customer feedback, SHP now offers information on a scale from 1 (lowest) to 9 (highest) of a patient’s risk score of hospitalization.
SHP’s new model presents home health agencies with the opportunity to review patient-level alerts and reporting and to adjust their Plans of Care (POC) based on the specific risk factors for each individual patient. This enhanced set of tools will significantly impact the way that agencies identify at-risk patients and help them in avoiding unnecessary and costly hospitalizations.
