The HRS platform now consists of three tiers: PatientConnect Complete, the original HRS solution that includes a tablet and Bluetooth peripheral devices; PatientConnect Core, a tablet only telehealth solution; and PatientConnect Mobile, a downloadable version of the HRS software that patients can use on their own devices.
HRS recognizes that while numerous patient populations can benefit from comprehensive telehealth solutions, not all patients require a full suite of peripheral devices. PatientConnect Core and PatientConnect Mobile offer all the benefits of PatientConnect Complete without the Bluetooth peripheral devices. Additionally, the introduction of PatientConnect Core and PatientConnect Mobile offer lower cost alternatives to PatientConnect Complete.
These new offerings allow organizations to reach more patients with telehealth, including behavioral health patients, chronic care management patients, palliative care patients, and patients that benefit from advanced symptom management. The three tier approach is supported by HRS’ expanded disease modules, which now include over 30 categories, such as behavioral health, palliative care, smoking cessation, and several new post-surgical care plans.
In addition to using the HRS tiers for specific patient populations, patients can use the three tier approach as a gradual path to independence. Once high-risk patients are ready to move off PatientConnect Complete, they can be placed on the Core or Mobile solutions. This approach allows high risk patients to slowly build independence, while still having the peace of mind that comes with remote patient monitoring.
Patients using PatientConnect Core and PatientConnect Mobile will have access to the HRS software features available on PatientConnect Complete, including disease specific educational videos, teach back quizzes, video visits, daily health questionnaires, medication reminders, custom surveys, and wound imaging. Clinicians also have the ability to contact patients using text, call, or multi user video conferencing.
HRS is continuously improving upon its solutions to enhance patient care and improve accessibility to remote patient monitoring.
