BALTIMORE, Oct. 3 /PRNewswire/ -- XLHealth announced today that it has changed the name of its Health Support Plus plan to Care Improvement Plus. The company had recently announced that CMS had granted it approval to market its new plan, which focuses on the unique healthcare needs of 98,000 chronically ill Medicare beneficiaries living in Maryland.
The new plan, now named Care Improvement Plus, will provide members with disease management services, a Medicare Part D drug benefit and medication management services. To join, members:
- must be eligible for Medicare - have diabetes, CHF and/or ESRD - reside in one of the plan’s service areas: Baltimore City and Anne Arundel, Baltimore, Carroll, Harford, Howard, Montgomery and Prince George’s counties
Care Improvement Plus’ enrollment begins November 15, 2005. Members joining between then and December 31, 2005, will begin receiving plan benefits on January 1, 2006.
