Diabetes Care Partners® and L.A. Care Health Plan Sign Multi-Year Diabetes Collaboration Deal
LOS ANGELES, March 1, 2018 /PRNewswire/ -- Diabetes Care Partners® announced today that it's expanding its collaboration with L.A. Care Health Plan, the nation's largest publicly operated health plan. In a multi-year agreement, Diabetes Care Partners® will deliver its Discovering Diabetes™ self-management program to L.A. Care members.
L.A. Care and Diabetes Care Partners® launched an outcomes-based pilot earlier last year. The pilot focused on providing access to diabetes self-management training and support throughout LA County, while increasing member engagement to manage diabetes through this unique program leading to decreased A1c.
The pilot showed increased engagement with a completion rate of 82% while decreasing A1c for high risk members by an average of 1.18. Lower A1c levels have been shown to reduce diabetes complication rates through conditions that include cardiovascular disease, blindness and neuropathy, while also substantially reducing medical costs. Reducing a person's A1c by 1 point can result in a decreased risk of complications by as much as 40 percent, according to a research study (UKPDS).
Managing diabetes can be complicated, but it doesn't have to be. In many instances, emotional stresses of the disease, along with myths and misconceptions about diabetes, can create barriers and hurdles for a patients' progress and wellness. Diabetes Care Partners® utilizes its Discovering Diabetes™ Program that addresses both the clinical and psychosocial issues patients face. The program is led by Registered Dietitians and Certified Diabetes Educators to empower patients focusing on healthy lifestyle changes such as nutrition, exercise, emotional wellbeing and weight management.
