Integrated network matches at-risk Ohioans with innovative prevention program
PHOENIX, Jan. 17, 2019 /PRNewswire/ -- Solera Health, a leading integrated community network, added Ohio-based Advanced Wellness to its national network of Centers for Disease Control and Prevention (CDC)-recognized diabetes prevention program (DPP) providers. One out of three American adults are living with prediabetes and 90 percent are unaware they are at high risk of developing type 2 diabetes. To combat this epidemic, diabetes prevention programs recognized by the CDC work with people with prediabetes on lifestyle changes that will reduce their risk of developing type 2 diabetes and improve their overall health. Advanced Wellness's DPP intervention focuses on ingraining a new lifelong lifestyle of healthy behaviors that include making good nutritional choices, being more physically active, and better managing stress. With a goal helping participants lose five to seven percent of total body weight, Advanced Wellness takes a personalized approach that includes individual goal setting and small participant groups to maximize time with trained lifestyle coaches, as well as one-on-one coaching. Moreover, Advanced Wellness gives its DPP participants a full year of free exercise in their on-site gym to complement their time in the program. "Solera Health recognizes the threat that diabetes poses to the health of our community," said Juli Robine, MPH, lifestyle coach and coordinator of Advanced Wellness's DPP. "As partners, they facilitate several processes including identifying those who can benefit from our program, allowing us to spend more time working with the participants to help them lead healthier lives." "Solera is committed to growing our national network of DPP providers who meet the needs of their local communities," said Brenda Schmidt, Solera Health's CEO. "It's a privilege to work with a program like Advanced Wellness that emphasizes personalized care as a way to effect real and lasting lifestyle change in Mantua area residents."
