BOSTON & SAN DIEGO--(BUSINESS WIRE)-- The Institute for Healthcare Improvement (IHI), a leader in health and health care improvement worldwide, is holding its Summit on Improving Patient Care in San Diego, California, taking place April 26-28. The conference focus is the transformation of the outpatient, primary care setting where clinicians and staff are developing new, team-based, whole-person approaches to physical, social, and behavioral health needs. Nearly 900 health improvers, health care professionals, and community change agents are gathered at this year’s #IHISummit to discuss and learn from efforts across the country to improve care for individuals and populations, while ensuring health professionals and staff have the tools and resources they need to implement these broadening goals.
IHI President and CEO Derek Feeley opened the Summit today by moderating a panel discussion featuring four innovative leaders who are confronting the challenge of improving primary care head on: Ann Lefebvre, MSW, CPHQ, Associate Director, North Carolina Area Health Education Centers Program, UNC Chapel Hill; Al Kurose, MD, MBA, FACP, President and CEO, Coastal Medical; Rachel Willard-Grace, MPH, Director, Center for Excellence in Primary Care, University of California, San Francisco, Zuckerberg San Francisco General Hospital and Trauma Center, Department of Family & Community Medicine; and April Kyle, Vice President of Behavioral Health, Southcentral Foundation. The discussion examined key aspects impacting the future of primary care, including understanding risk, using data effectively, sharing goals around patients’ well-being, reducing waste to free up more resources, and leveraging the unique power of teams to improve care and workforce joy.
Tomorrow, America Bracho, MD, MPH, CEO and President, Latino Health Access, delivers a keynote address about the need to work with patients and communities as partners. In a pre-Summit interview for the IHI Blog, Dr. Bracho previewed her remarks, commenting that one of the most important things for anyone’s health is to participate: “Participation is powerful on an individual level when you are more active with your physician, when you work together on a treatment plan … Universally, people see the power of participating and participating together. Participation is what transforms hope into things that are tangible.”
Pathways to Population Health (P2PH)
This year’s IHI Summit is also featuring the launch of Pathways to Population Health (P2PH), a multi-organization collaboration seeking to accelerate the efforts of health care organizations in making meaningful and measurable progress in population health. Funded by the Robert Wood Johnson Foundation, P2PH, part of the 100 Million Healthier Lives initiative, is currently welcoming additional participants to join the movement to improve health, well-being, and equity within communities. A special interest lunch event will take place during the Summit on April 28, and other tools and resources to support population health improvement efforts can be found on pathways2pophealth.org.
Beginning in June 2018, IHI will begin offering a free webinar series for interested participants to accelerate their population health improvement efforts as well as share with and learn from fellow change agents. Details can be found here.

Over the course of the three-day conference, dozens of general conference workshops, half-day IHI Quick Courses, and learning labs are organized across four key areas: People (workforce culture; joy in work; team-based care), Process (utilizing data; managing high-risk, high-cost patients), Partnerships (overcoming a fragmented health system), and Payment/Policy (value-based care).
