MINNEAPOLIS, June 21 /PRNewswire/ -- Cardiocom announces today that UCare Minnesota renewed their multi-year agreement with the Minnesota-based disease management company to provide systems for remote daily home monitoring of UCare’s Congestive Heart Failure (CHF) patients. UCare’s clinical studies show that the Cardiocom system improved quality of care and member satisfaction, and reduced inpatient admissions for CHF.
While there is no cure for CHF (affecting nearly five million Americans), accurate, daily biometric monitoring is critical to improve quality of life and prevent unnecessary inpatient admissions. To ensure that its members with CHF have an effective method of daily weight and symptom monitoring, UCare Minnesota has provided them with Cardiocom’s interactive monitoring devices.
“We are honored to continue our relationship with UCare Minnesota to manage their heart failure members,” said Daniel Cosentino, president of Cardiocom. “In the past three years, Cardiocom’s daily home patient monitoring and management system has improved the quality of life for their members, reduced hospitalizations, and facilitated efficient and targeted disease management. We look forward to continuing this mutually beneficial relationship.”
