ActiveHealth Management Partners With XLHealth To Deliver Health Management Services To Tennessee Medicare Beneficiaries

NEW YORK, Dec. 14 /PRNewswire/ -- ActiveHealth Management, Inc. (ActiveHealth) today announced that it has entered into a partnership with XLHealth to provide health management services to seniors in Tennessee. Through XLHealth’s recently awarded prime contract under the Chronic Care Improvement Program (CCIP), ActiveHealth will assist XLHealth in its unique approach for providing disease management services to physicians and their patients.

ActiveHealth’s proprietary CareEngine(R) System is providing clinical decision support to assist XLHealth in improving the quality of care for Medicare beneficiaries who qualify for CCIP. This is part of a new relationship in which ActiveHealth supports XLHealth in identifying opportunities to improve care for patients with chronic conditions who are most in need.

Administered by the Center for Medicare & Medicaid Services (CMS), the CCIP pilot program awarded to XLHealth will allow the company to apply the beneficial features of its unique disease management services to traditional fee-for-service Medicare beneficiaries with certain chronic conditions, such as diabetes and congestive heart failure. The initiative is designed to help improve health outcomes for these Medicare recipients, reduce medical costs, and improve participant and medical provider satisfaction. The awards are a part of CMS’s implementation of the Medicare Modernization Act of 2003.

Participation of Medicare beneficiaries in the CCIP is voluntary, and all services will be provided to participants at no charge. Additionally, the program does not change participants’ access to traditional fee-for-service Medicare benefits.

Designed to improve quality of care and thereby reduce costs, ActiveHealth’s patented CareEngine system uses a proprietary clinical technology platform to apply a comprehensive set of evidence-based clinical rules to aggregated patient medical claim, laboratory and pharmacy data. It then generates patient-specific treatment improvement suggestions called Care Considerations that are communicated to physicians and their patients to help prevent avoidable clinical complications and hospitalizations. These services have been shown to significantly improve clinical outcomes and reduce medical costs across a covered population. The CareEngine system currently provides coverage for more than 6 million commercial, Medicare and Medicaid lives.

“We’re excited to be partnering with XLHealth in delivering a program that we believe will positively impact Medicare beneficiaries who are part of a population that can benefit greatly from disease management,” said Lonny Reisman, MD, CEO of ActiveHealth. “This initiative is another application of the ActiveHealth health management solutions that support XLHealth’s unique approach to disease management for physicians and their patients with chronic conditions such as diabetes and congestive heart failure,” he added.
