LONG BEACH, Calif., Oct. 27 /PRNewswire/ -- Molina Healthcare, Inc. announced today that it is expanding its disease management efforts in partnership with Pfizer Health Solutions Inc (PHS). Molina will work with its members -- individuals covered under Medicaid and other government-sponsored health insurance programs -- to help improve their health and manage their chronic diseases, including asthma, diabetes and heart disease. Molina’s care managers will use PHS’ care management technology to provide patient-centered care to Molina members.
Through this program, all members of a health care team (patient, physician, care manager and administrator) work together to create a tailored care plan for each patient. This coordinated approach includes encouragement and support of the patient’s health through phone calls and educational mailings. For example, care managers work with members to follow physician guidelines, including those for medication, diet and exercise.
