We are currently searching for an Advance Practitioner to provide direct patient care services evaluating and managing patients with complex vascular diseases in support of research by the Division of Intramural Research in the National, Heart, Lung, and Blood Institute (NHLBI). This opportunity is full-time, and it is on site in Bethesda, MD.

Duties & Responsibilities
  • Manage patient care that is delivered in accordance with policies, procedures, and standards of practice within the guidelines of the Maryland Board of Physicians and the Medical Board of the Clinical Center of NIH.
  • Provide care to acute and /or complex patient populations in the inpatient and outpatient settings.
  • Identify and communicate patterns and trends that may predict the direction of illness and provides nursing care to prevent adverse outcomes.
  • Independently provide vascular medicine evaluation and care of adult and pediatric patients admitted to the inpatient service, the outpatient clinic, or other Clinical Center service in consultation; keep complete, accurate medical documentation.
  • Provide patient with vascular domain-specific instructions and counseling; inform physician of observations, problems, or health concerns relevant to the patient.
  • Interview patients to obtain medical history, history of use of medication, previous related symptoms, and non-related diseases. Document all information and observations.
  • Assess patients in diverse outpatient and inpatient settings for vascular disease manifestations, evaluation, and management, keeping accurate records and informing physician of patient’s status and communicating with primary teams.
  • Assess patients to determine the need for medical attention and orders special screening and developmental tests and lab testing; evaluate patients and provide specialized care before, during, and after vascular testing. Keep accurate records and inform physician of patient’s status.
  • Review and interpret laboratory and other diagnostic test data and documents inpatient and outpatient chart or record; actively participate in all clinical rounds and conferences; educate patients and families about diseases, and identify normal/abnormal findings on history, physical exam, clinical lab studies, vascular imaging, and functional assessment studies.
  • Participate in the development of new vascular medicine clinical management protocols, assist in implementation, and collect and analyze data to measure outputs.
  • Participate in the operations of the vascular diagnostic/functional imaging center, evaluate patients undergoing vascular studies and interpret results where appropriate.
  • Complete medical records (admissions, discharges, progress notes, consult reports) in a timely and effective manner.
  • Communicate verbally and in the medical record of patient management recommendations of the attending physician.
  • Efficiently orchestrate daily routines in clinical practice.
  • Contribute to the interdisciplinary team as evidenced by actively participating in unit rounds/meetings, making referrals to address patient care needs, thoroughly communicating the plan of care and the patient’s response to care.
  • Communicate with patients and their family members in a manner that is perceived to be therapeutic, timely, and accurate.
  • Provide verbal and written education to patients and their family members as evidenced by individualized teaching, use of available resources, and modifying teaching methods to achieve the highest level of learning.
  • Advocate for patient and families and support patient rights.
  • Practice and enforce responsible safe clinical and laboratory practices; follow established safety guidelines and disseminate safety standards to all staff.
  • Incorporate patient safety principles into patient care.
  • Oversee adult and pediatric patients in a rapidly growing vascular medicine clinical research program.
  • Perform inpatient and outpatient clinical consultations including patient assessments, orders, and reviews clinically appropriate lab and imaging studies; coordinate input from consultants; and document care in the inpatient chart.
  • Attend clinical and research conferences designed to educate the team and optimize patient care and research.
  • Utilize behaviors and best practices for a specialty area. Proficiently:
    • Consult with physicians, nurses, and community-based health care personnel, communicating current data resulting from clinical trials and the management of designated patients.
    • Recommend approaches and solutions to problems that may arise.
    • Integrate evidence-based practice into clinical practice and evaluates patient outcomes.
    • Contribute to the establishment and upgrade of standards and policies and interpret the significance of research findings to applied clinical settings.
    • Seek new knowledge and skills based on self-assessment, feedback from federal task leads and staff, and changing clinical practice requirements.
    • Contribute to the identification of learning needs for the practice environment and provide learning opportunities for others as assigned.
    • Assist in the development of a vascular medicine educational curriculum for clinical providers, researchers and trainees.
    • Recognize and resolve significant issues and keep federal task lead (FTL) appraised of progress and/or changes in the patients’ conditions and medical treatment plans.
    • Work with staff physicians, members of the clinical care team, and other individuals involved in direct patient care to ensure that the requirements of research protocols are carried out; contribute to overall team strategies, suggest ideas and suggestions for modifying and improving methods or techniques; demonstrate comprehensive understanding of research objectives.
    • Provide appropriate evaluation and management of treatment plans, suggest alternative treatment regimens when appropriate. Skilled in the use of emergency procedures.
  • Stay current with knowledge of the biomedical research process, support of patients on biomedical research protocols and awareness of ethical issues.
  • Study and evaluate literature related to vascular medicine research, laboratory clinical projects; incorporate new medical and research techniques and approaches as they become available; participate in continuing education activities; and maintain necessary credentialing through required continuing education to maintain professional licensing.
  • Participate in the development of clinical research protocols, providing expertise in experimental design relevant to practical patient care.
  • Provide technical expertise for substantial and effective contributions to research through scientific discussions with staff and collaborators.
  • Assist physician during minor research procedures.
  • Perform minor procedures.
  • Perform clinical research imaging and functional assessment procedures
  • Contribute to the development and maintenance of research databases, including data entry.
  • Contribute to research goals by collaborating with physicians, multidisciplinary team members, data managers, and statisticians to obtain, collate, and organize data. Contribute to data analysis and interpretation. (Co-)Author scientific papers for publication in recognized journals, present research data in group settings or at meetings or seminars.
  • Identify and communicate the impact of the research process on patient care, adjust interventions based on findings and reports issues/variance promptly to the research team.
  • When appropriate or requested, administer research interventions, collect patient data according to protocol specifications, and evaluate patient’s response to therapy; respond to variances in protocol implementation; and report them to the research team.
  • Works with federal task lead and staff to resolve ethical conflicts/issues that may arise in a clinical research setting.

 

 

Requirements
  • Maintain a current and active certification as a Nurse Practitioner or Physician Assistant.
  • Bachelor’s degree in related field, Masters preferred.
  • Minimum of 5 years of applicable experience
  • Must be able to be credentialed by the NIH.
  • Knowledge pertaining to privacy of the patient, privileged information, and secure handling of the patient’s medical records.
  • Knowledge of federal regulatory requirements pertaining to clinical research.
  • Knowledge of Good Clinical Practice (GCP).
