Molina Healthcare Enhances Disease Management Program

LONG BEACH, Calif., Oct. 27 /PRNewswire/ -- Molina Healthcare, Inc. announced today that it is expanding its disease management efforts in partnership with Pfizer Health Solutions Inc (PHS). Molina will work with its members -- individuals covered under Medicaid and other government-sponsored health insurance programs -- to help improve their health and manage their chronic diseases, including asthma, diabetes and heart disease. Molina's care managers will use PHS' care management technology to provide patient-centered care to Molina members.

Through this program, all members of a health care team (patient, physician, care manager and administrator) work together to create a tailored care plan for each patient. This coordinated approach includes encouragement and support of the patient's health through phone calls and educational mailings. For example, care managers work with members to follow physician guidelines, including those for medication, diet and exercise.

"Molina is implementing this national program to help its members take a more active role in managing chronic disease and preventing complications from these conditions," said Dr. Richard Helmer, vice president and chief medical officer of Molina Healthcare. "We feel that high quality disease management is particularly important for the members we serve -- people who have traditionally faced barriers to accessing quality healthcare."

"Molina and PHS are collaborating to provide access to new health- improving resources and supportive clinical technology," said PHS Vice President John Sory. "We are dedicated to working together to reach Molina's members who could benefit from additional care -- and to showing that reaching out to care for patients yields great benefits to all."
