VisualMED Clinical Solutions Corp. and Intelaform Expand Partnership to Address Hospital Readmission Reduction Transition Care Programs in National Capital Region

VisualMED Clinical Solutions Corp. ("VisualMED") (PINKSHEETS: VMCS) (FRANKFURT: VA6) and Intelaform, Inc. ("Intelaform") announce an expansion of their Maryland transitional care initiative to include the District of Columbia by forming the National Capital Region Hospital Readmission Reduction Consortium ("NCR Consortium"). The Consortium will focus on implementing community-based continuity of care programs targeting Senior Citizens. The objective is to decrease hospital readmission rates for the most common conditions: cardiovascular, respiratory and diabetes. VisualMED and Intelaform welcome other companies in joining the NCR Consortium to assist the National Capital Region.

As of August 2012, none of the organizations participating in the Centers for Medicare and Medicaid System ("CMS") Community-based Care Transition Care Programs ("CCTPs") were involved in the District of Columbia and Virginia regions. The Community-based Care Transition Program (CCTP), created by Section 3026 of the Affordable Care Act, and administered by CMS does test models for improving care transitions from the hospital to other settings and for reducing readmissions for high-risk Medicare beneficiaries. Up to $500 million in total funding is available from 2011 through 2105.

There are no such CCTP programs to date in the National Capital Region to assist hospitals in reducing readmission rates for the three most common disease categories. VisualMED and Intelaform have created the NCR Consortium in order to leverage their expertise to fill the need. The leaders of the NCR Consortium, VisualMED and Intelaform will combine their expertise in medical workflow processes, strong knowledge of public health issues, and their relationships in the region, in order to help District of Columbia and Virginia hospitals meet public policy objectives.

VisualMED and Intelaform believe their product offerings are "Best of Breed" and can be leveraged into the deployment of a comprehensive care transition solution that incorporates community-based organizations (CBOs) and acute care healthcare facilities as partners. VisualMED and Intelaform products and their working knowledge of multiple care settings, will allow the implementation of a cost effective continuity of care program to reduce hospital readmission. We also have relationships with suppliers of intelligent home monitoring devices.

Reducing hospital readmission is not just an opportunity in the United States. Intelaform is also actively helping VisualMED in submitting a $20M Transitional Care program to the newly elected government of the Province of Quebec, and to the Canadian Federal Government. There are currently no transitional care programs to prevent costly readmissions anywhere in Canada at this time. The new provincial governing team has a long history of innovation in improving healthcare delivery and patient outcomes.

Reducing hospital readmission is a huge opportunity. One in five Medicare patients returns to the hospital within a month of discharge. Nearly 6 million patients are readmitted within 30 days of release each year, costing Americans some $47.5 billion in additional hospital bills. This national average has remained steady at around 19 percent for several years.
